Job Summary Join UT Southwestern as a Lead Transplant Coordinator (RN) at Clements University Hospital within our Lung Transplant Clinic and help with our commitment to deliver compassionate care and all-around excellence. As a Lead Transplant Coordinator at our Magnet-designated hospital, you will support the Lung Transplant Clinic by providing excellent patient education for the patient and their families, triaging, and supporting the clinic. In this position you will also help develop patient calendars detailing all their appointments to help with their transplant transitions. Experience and Education Bachelor's degree in medical field required. Bachelor's degree in Nursing preferred. Three - four years of Medical/Surgical or Intensive Care experience required. 7 - 9 years of clinical transplant experience preferred. Job Duties Collects relevant clinical data using a systematic and continuous process. Interprets tests and procedures based on established evidence-based standard of care. Anticipates patient's and/or patient's family's expectations and needs and utilizes them in conjunction with assessment to develop appropriate plan of care for patient. Synthesizes data and patient information relevant to organ specific transplantation. Utilizes critical thinking skills in planning and prioritizing nursing care. Identifies and implements appropriate nursing interventions based on assessment. Completes documentation of interventions. Coordinates interdisciplinary plan of care. Communicates plan of care with family, patient, and transplant team members. Prioritizes nursing interventions based upon assessment. Identifies changes in condition as evidenced by assessment, labs, x-rays, etc. Reports changes and takes follow-up action as appropriate. Evaluates effectiveness of planned strategies in relation to the patient's attainment of expected outcomes. Uses ongoing assessment data to revise the plan of care and expected outcomes. Documents interventions and modifications to plan of care. Adheres to appropriate policies, procedures, clinical pathways, and nationally accepted standards of nursing practice while delivering care to patients. Suggests practices to enhance nursing care based on current literature and research. Provides transplant specific health teaching that addresses patient self-monitoring and promotes self-care. Completes documentation of all education provided. Obtains feedback and evaluation of understanding from patient and family. Demonstrates effective communication with children, adolescents, adults, and older adults. Assists the patient through the University system to facilitate the patient treatment plan. Develops patient calendars detailing all transplant and/or outpatient appointments for transplant services. Interfaces with other hospital departments and physicians to facilitate testing and/or the admission process. Mentors staff and educates other depts to facilitate compliance with transplant regulatory policies related to organ allocation, candidate listing, and ABO and UNOS ID verification for living donors and/or recipients. Advocates for the patient's medical & legal rights & promotes the informed consent process. Coordinates Selection Committee(s) & clinical policy/guideline review/revision to ensure adherence with evidence-based medicine & regulatory requirements. Promotes inter and intra dept relationships. Staffs' area monitors all Human Resource functions. Reviews workflows on an ongoing basis to eliminate waste and improve efficiency. Works with staff to identify and achieve cost savings. Active participant in QA/PI activities. Monitors progress looks for trends. Assists Quality Manager/Team with development, implementation & evaluation of action plans. Attends at least 80% of scheduled staff meetings & 100% of mandatory education sessions. Structures a learning environment & makes time for staff to attend education activities on campus. Develops an orientation plan for each new employee, provides coaching for preceptors & monitors staff progress & documentation of competency achievement. Provides coaching throughout the year and conducts annual performance reviews & goal setting. This position is security-sensitive and subject to Texas Education Code §51.215, which authorizes UT Southwestern to obtain criminal history record information.
